starting a map completely from scratch

The title says it all....what is the best way to build a map completely from scratch? I want to build a fictional island, but have yet to find a suitable island elsewhere to use as a starting basis for it.

I have downloaded/read that tutorial and he pulled the heightmap data from the microdem program?
Actually may have thought of a way to make a greyscale map using the editor for battlefield 1942 (can save the heightmap of a map in a 24bit greyscale)
