Makes me think of :
What do you mean when you write...
if I take out the 4/3 support and get the distorted image

There's nothing to take in or out honestly.

What i mean is: in order to get the 4/3 support you need to activate this feature in jsgme. But if i launch the game without activating this feature using the normal up3.4 setup wide then i get the normal joystick control page

Just have a look at it but most likely is just a bug in my set up I have some weird things happening and several crashes and BSD caused by faulty ATI drivers install so it is possible that I ended up with some corrupt files

Looks like indeed it's a special thing happening on your end.
I have just cross-checked this on 3 different systems with "Narrow Screen (4 by 3 Format)" mod enabled in JSGM and screen resolution set to 1280x960, all of the tests went well.

Yep in fact this morning is working normally I tried again after the last exchange and everything is back to normal also with the 4/3 support mod enabled!!!.  however because my system like to challenge me now I have a sound bug on the Cyclone engine that suddenly lost its sound and being the B-239 one of my favorites I have a new problem to solve.
