I am terrible at sword fighting.... 

Exactly for this reason I became an archer in this game.
And a good horseman.
My combat perks: sword: 6, bow: 17; skill riding: 19.
Highest level is 20.

You can do a complete runthrough as an archer.
You're not forced at anytime to use the sword.
Oh wait, one time you have to do a training
with Bernard, who is the best swordfighter in the game btw.
And he beat me so hard that I decided to try the bow instead. 

So try again. And enjoy it as I do.

How big is the complete playable map?

Not at all, its a very small one, only 16km² (4x4km) when iirc.
I have all this large open world games (apart from Far Cry 5) with way more area,
but there most of the stuff is generated. In this KCD map everything is handmade.
After 189 hours playing it there are locations I didn't know before and just came across yesterday. 

This is a map wit all places available, I painted a red circle to the "interesting site" just to show the symbol
and how many to find in the world...

a ride from ie Rattay to Uzhitz and then to Talmberg tooks a least a ingame day, when you survive .. 
there are many brigands, bandits and cumans in the woods and the roadside bushes waiting for you.

No proper bathrooms or toilets. No bath-tubs to take a bath. No soap. Proper bathing was only done around
Christmas time to purify the body, the rest of the year you just wash in the river.

You can visit the "bathhouse"! You can go there whenever you need it...
costs 10 to 40 Groschen only. btw, I have 9000 Groschen atm in my
second game runthrough at the beginning, in my first completed runthrough
I had ~ 150k Groschen ... so yes, to have a "bath" there is affordable. Every day.
